This chronic disease, in which the immune system destroys the insulin-producing cells of the pancreas, is affecting an increasing number of children and adolescents. Between 2012 and 2021, nearly 5,800 new cases were recorded in this German region, where two million young people under the age of 20 are monitored.

Among girls, the incidence rate increased from 23.5 cases per 100,000 inhabitants in 2012 to 32.5 in 2021, representing an average annual increase of 3.7%. Among boys, the increase is even more pronounced, with the rate rising from 25.2 to 38.9 cases per 100,000 inhabitants, an annual increase of 5%. The sharpest rises are observed in boys aged 0 to 4 and girls aged 15 to 19.

Researchers also noted that the average age at diagnosis has decreased. In 2021, boys were diagnosed on average 1.6 years earlier than in 2012, and girls 0.5 years earlier. This could indicate either an earlier onset of the disease or improved screening.

The COVID-19 pandemic period, between 2020 and 2021, was analyzed to determine if it had any impact on this trend. Although no clear change in progression was detected in either sex, a sudden increase in cases among girls was noted at the beginning of 2020. However, this increase remains difficult to interpret due to the short observation period and wide confidence intervals.

The reasons for this continued rise in type 1 diabetes among young people are not yet clearly established. Several factors could play a role, such as changes in early childhood diet, weight gain, or maternal factors. The pandemic may have also influenced these trends, particularly due to lifestyle changes related to lockdowns. Increased weight gain was observed among children and adolescents during this period, which could accelerate the onset of the disease. Additionally, a decrease in respiratory infections, typically common among young people, was noted. This could have a dual effect: reducing virus-related cases or, conversely, increasing incidence due to reduced exposure to these pathogens.

This study highlights the importance of continuously monitoring the evolution of type 1 diabetes among young people. A better understanding of the underlying causes of this increase would enable the implementation of effective prevention strategies. In Bavaria, as in other regions of Europe, this disease represents a growing challenge for healthcare systems, with potential repercussions on the quality of life of young patients and public health costs.

Source Credits

Primary Source

DOI: https://doi.org/10.1007/s10654-026-01417-y

Title: Increasing incidence of type 1 diabetes in children and adolescents from 2012 to 2021 in Germany: trends before and during the COVID-19 pandemic

Journal: European Journal of Epidemiology

Publisher: Springer Science and Business Media LLC

Authors: Carolin T.
